Thank you for your inquiry regarding Group Health Cooperative of South Central Wisconsin (GHC-SCW)’s Conversion Plans.
If you are residing in Dane County or a contiguous county, you are eligible to apply for an Individual Plan. GHC-SCW’s geographic service area is Dane County, Wisconsin. If you reside outside of our service area, you may be eligible for coverage provided your residence is located in one of the following contiguous counties: Jefferson, Green, Lafayette, Rock, Columbia, Dodge, Iowa or Sauk.
The Conversion Plan Option is available to you if GHC-SCW receives the application within thirty-one (31) days after your group coverage ends. This conversion will only be allowed if your GHC-SCW enrollment continues with no lapse in coverage. Note that there is no coverage for prescription drugs with the Conversion Plan.
How to apply
You may apply for the Conversion Plan option if you meet the application timelines. If your application for the Individual Plan is not approved prior to your group coverage ending, you may convert to the Conversion Plan until the Individual Plan application is finalized, so there is no lapse in coverage.To apply for the Conversion Plan, please complete the following forms:
