Wellness Reimbursement Program

Community Supported Agriculture, or CSA, is a unique opportunity to investigate the great taste of fresh, local foods. CSAs allow you to support local farmers by purchasing a share of food from an area farm on an annual basis. In return, you will share in the harvest! On a predetermined delivery schedule, CSA shareholders will pick up a box of in-season produce. Often recipes will be included to assist you on your healthy adventure!

To learn more about Community Supported Agriculture or to sign up with a local farm, please visit the Madison Area Community Supported Agriculture Coalition Web site. The harvest season typically runs from late May to October.

To request reimbursement for your CSA purchase, fill out and print the Wellness Reimbursement Form. Each individual farm will provide a membership request form.  Send a copy of the completed membership form along with your Wellness Reimbursement form to GHC-SCW.  GHC-SCW will check monthly with your farm to verify membership. Once verification occurs, within 45 days, you will receive your reimbursement.

The deadline to submit reimbursement requests is December 31 of each calendar year. You must include your full name, GHC-SCW member number, date of birth and address to be eligible for reimbursement.
