If you are residing in Dane County or a contiguous county, you are eligible to apply for an Individual Plan. Group Health Cooperative of South Central Wisconsin (GHC-SCW)’s geographic service area is Dane County, Wisconsin. If you reside outside of our service area, you may be eligible for coverage provided your residence is located in one of the following contiguous counties: Jefferson, Green, Lafayette, Rock, Columbia, Dodge, Iowa or Sauk.

How to Apply
To apply, please complete the following forms:
Please note, applicants may be rejected if their health status does not meet GHC-SCW's medical underwriting criteria. If medical records are needed to further determine acceptability, you will be asked to furnish them at your own expense.
The application process for the Individual Plan generally takes one to three weeks. We will promptly notify you regarding your rejection or acceptance into the Individual Plan.
If you are a current GHC-SCW member through an employer group or COBRA/Continuation, and your application is rejected, you may still apply for the Conversion Plan option if you meet the application timelines. If your application for the Individual Plan is not approved prior to your group coverage ending, you may convert to the Conversion Plan until the Individual Plan application is finalized, so there is no lapse in coverage.
Payment Method
Please submit your payment for the first month's coverage along with your Application, Rate Worksheet and completed Health Questionnaire. If you are not accepted for coverage, your payment will be returned.
You may pay with a personal check, or money order. If you would like to preauthorize your monthly payments, please complete the Authorization Agreement for Preauthorized Payment. GHC-SCW will still need a check for the first month's coverage.
